Pet vaccination certificate format (what to include)

Owners ask for a vaccination certificate for boarding, grooming, travel and housing societies. A clear certificate saves them a trip back to the clinic and shows that your records are in order.

What the certificate should show

  • Clinic name, address, phone and the veterinarian's name and registration number.
  • Owner's name and contact details.
  • Pet's name, species, breed, age or date of birth, sex and colour or identifying marks.
  • For each vaccine: name, manufacturer, batch number, date given and next due date.
  • The veterinarian's signature and the clinic stamp.
  • The date the certificate was issued.

Why the batch number matters

The batch number lets the clinic trace the exact vaccine given, and is often requested for travel and boarding. Recording it at the time of vaccination is far easier than reconstructing it later.

Make it from the record, not by hand

If every vaccination is recorded on the pet's profile with date, vaccine and batch, a certificate is a printout of data you already have. If records are on paper, certificates are copied by hand and errors creep in.

Questions

Is a vaccination certificate valid without the vet's signature?

Most places expect a signature and clinic stamp. Add both before you hand it to the owner.

What does an airline or hotel look for?

Usually the rabies vaccine date and next due date, the pet's identity, and a signed record from a registered veterinarian.

Should the next due date be on the certificate?

Yes. It helps the owner plan the next visit and shows the pet is up to date.
